North Korea launched at least one missile (and possibly more) over North Japan, specifically, the island of Hokkaido.
Here, in red, is Hokkaido, a very populated island (5+ million) almost directly connected to mainland Japan:
This is a belligerent Act of War. To put it in more relatable terms, it is if I pulled a pistol, aimed it in the general direction of your head, and fired -- purposefully missing by twelve inches.
There are rumors that FatBody Jong-un is having some internal power struggles, and is purging his capitol city of people.
But whatever the situation in Pyongyang, the truth of the matter is that North Korea just committed a true Act of War.

Answer: Not every time. The current technology isn’t perfect, so I just imagine we don’t want to risk taking a shot and missing at this point in time. Getting one in early boost phase would be better, but to do that we would have to shoot it over NK.

Give Japan the bomb.
Believe it or not, Japan detonated a small atomic bomb months before the USA did. They tested their atomic bomb in February 1945 at Hungnam, North Korea.
We didn’t test our first bomb until July 16, 1945.
The zinger is that they used up all their fissionable material in the first test and had nothing left for a weapon.
Our leaders should give nuclear weapons to South Korea for sure, and to Japan, if enough Japanese leaders want them.
