Too much automation in the cockpit. Pilots seem to be taught to manage systems more than actually flying the plane. The pilot flies the computer while the computer flies the plane.

I just saw a program on “The Smithsonian” channel about China Air 006. The 747 fell from 41,000 feet to near sea level rolling and turning the whole way.
They eventually determined it was pilot error after losing one engine.
Interestingly the pilots did an incredible job of saving the plane as it literally had large parts of the wings and fuselage torn off. They had to lose altitude by adjusting speed and turn using other means than normal.
They ended up making an incredible landing.
The pilot had thousands of hours flying in 747s and they think he thought his instruments had gone out because the plane was doing things it couldn’t do. It turned out the instruments were correct.
He must have made the wrong turn over Macho Grande.
