The throwing star, or shuriken, is a cute novelty weapon, but to my knowledge, nobody has ever been killed by one outside of the movies.
In actual practice, throwing a pebble is far more dangerous, and many people have been killed by them. Some martial arts systems actually teach pebble throwing as pebbles are ubiquitous. Of course it helps to have a pitching arm like Nolan Ryan and a lot of practice.
