That was my first impression. Also looks like a meshed split thickness skin graft to close the wound distally, which would point towards trauma of some sort.

Maybe it’s varicose veins and not a scar. They can make people’s legs look pretty bad. He’s a heavy smoker, so it wouldn’t surprise me if he had varicose veins.

That’s not a scar on his head. It’s a part. Look carefully at the direction of the hair on either side of the “scar”.

Trust me. I have the same kind of hair. That’s what it does, if you’re not careful when you brush it.
