No if there is no winner after 90 minutes then there is a max. 2* 15 minutes extra time. Confusing. Does a team win if ahead after the first additional 15 minutes, or do they always play two additional 15 minute periods?

They tried that around 2000 (e.g. Euro 2000), but it didn't produce the desired results (instead of playing offensively, many teams played defense as not to concede a goal). So they switched back to the tried and true traditional format with 2x15 minutes of extra time.

The referee adds time he thinks has been wasted. Usually between 1-4 mins but can technically be any amount.
