No if there is no winner after 90 minutes then there is a max. 2* 15 minutes extra time. Confusing. Does a team win if ahead after the first additional 15 minutes, or do they always play two additional 15 minute periods?

No. What you mean is a "silver goal", which is a variant they tried after the golden goal didn't work as expected.
