There is no reason to continue containing particles which have already fused.

As long as they are going in the right direction, sure... The problem with a cyclic reaction is that the angle of incidence will vary with every reactive fusion event. Having a ton of neutrons come blasting through the SIDES of your motor isn't very efficient.

Bussard can claim eleventy-first power ranges. There is a lot of healthy skepticism on that board and some detailed reasons for it that make a certain amount of sense. Of course, if just anyone could figure it out... they probably would have by now. I'll see if I can't dig up another source on Bussards work. Give it a once over. Not saying I'll understand too much, without a bit more math, but the basic principle is familiar.

No. The "vessel" in which the fusion is designed to occur is a cage of electrons. Since it is formed electrostatically, it reforms from moment to moment. It should be possible to operate in almost steady state mode under normal circumstances.

The overall design is particularly elegant and simple. A magnetic cage is constructed of simple circular magnetic coils joined as the faces of a polyhedron. Magnetic forces constrain electrons to accumulate in the interior. The attraction of the electrical charge of this virtual cathode draw positive ions to the interior of the electron swarm, where they oscillate back and forth depending on the energy of the system.

If things get energetic enough, they will collide, and hopefully fuse.

The advantage of this arangement is that it requires little in the way of start-up energy, so it shouldn't take long for even low levels of fusion to exceed "break-even" levels of energy production.

The discussion of how to make the fusion chamber into a power source for space flight and into a rocket engine of great power and efficiency may be geting ahead of the game a bit, but that too is rather interesting. Doctor Bussard is clearly a very interesting fellow.

Bussard's ramjet was a quite interesting element of Niven's "Known Space" (?) stories...
