In this Vlog Bill plays a video made by a frontline ER Doctor in NYC. He says the standard ventilator treatment being used across the world is not helping any of his patients. In fact, he says it's making things worse. And, he goes into detail as to his observations. And, then pleads for more frontline doctors to speak out.

Trump spoke about HCQ as if it was widely available. IMHO there seems to be a disconnect somewhere between her hospital in Queens and the White House.

Up through about the day before yesterday my subjective impression was that there seems to have been a studious avoidance about mentioning HCQ on CNN in any circumstance (as if someone had sent a memo to banning its mention on the air). CNN went through an entire interview of CNN medical correspondent Dr. Sanjay Gupta and then Covid-19 positive/self-quarantined Fredo without mentioning HCQ at all. Would not more information on HCQ advisability versus availability be newsworthy for the epicenter of the Covid-19 epidemic? what bout the dangers of transmitting across state lines, the additional burdens to family members of 24/7 monitoring stricken elderly relatives, the potential for the unintended consequences of state policies engendering black markets... (somewhat rheorical... may not be understanding something...)

I read that HCQ is a zinc ionifier that allows zinc to be absorbed by the cells. And that zinc is what interferes with the virus replication.

Quercetin is a natural zinc ionifier.

If the medical community is giving up on the father, zinc + quercetin supplements might help. Both are over the counter.

Well I’m no expert.

I’m going by one article that said HCQ worked by being a zinc ionifier. And since HCQ is prescription, I googled “natural zinc ionifier” and quercetin popped up.

No, the problem is that Cuomo's order explicitly prohibits NY pharmacists from dispensing it for patients who are not hospitalized or in the formal trial. So the PCP can write a prescription but it cannot be filled in NY. Fortunately it sounds like a NY doctor's prescription can be filled by a PA pharmacy.

And that therapy is too late for critical patients. This is insanity. We’re talking about a med regimen that was under $15 ante. We ought to be able to come up with the few hundred thousand cycles required, no problem. Instead we have moron governors limiting docs writing Rxs.

He (and others reading this) might have gotten it faster from an older relative with RA. My mother has it and takes hydroxychloroquine, and if anyone in my family gets it I WILL get the 10 or 12 pills to effect a cure, whether the doc likes it or not (and I will fire him if he doesn’t).

Don't know if you've noticed, President Trump is attacking cuomo at most of his pressers. He knows what's going on in NY. What he said today about HCQ was a direct message to "Dr"cuomo. "Release HCQ!" He's giving "Dr"cuomo a chance to change his ways or laws. Watch what "Dr"cuomo says tomorrow at his bloviating daily pressers.

“We continue to study the effectiveness of hydroxychloroquine and other therapies and treatments of the virus,” Trump said on Friday at the White House press briefing. “And we will keep the American people informed of our findings.”

Trump continued, apparently off-script from his prepared remarks, “Hydroxychloroquine, I don’t know. It’s looking like it’s having some good results. I hope that would be a phenomenal thing.”

This is the continuation of a glaring messaging gap within the administration. Trump has breathlessly promoted the drugs while the public health officials are cautiously waiting for scientific evidence, and tamping down expectations, by saying that the information out there already is only anecdotal and not proven on a large scale.

A report from the front line

—Hospital still empty
—Death rate of vented patient is NOT anywhere near 80%, at least in my institutions.
—Patient ARE being started on hydroxy and azithromycin at the confirmation of disease, not waiting until vent dependent
—I continue to see excellent results with hyrdoxy and Zithromax
—I am hopeful that we are turning the curve
—I have been doing a lot of work with telemedicine to relieve some in the hot zone, so I haven’t posted an analysis, will try to do so soon (foreshadow, data is holding)
—I am hearing more talk of perhaps getting the country reopened, I think April 30 we will start
—Peak net week in midatlantic (8 April NJ, 9 April NY) Lets see what happens

All in all, we will get through this, we will survive, and hopefully we will not freely give up our liberties.

Oh — Blood transfusions are universally bad — unless you really really need them. Lots of gold standard studies demonstrating restrictive transfusion practices are superior in terms of survival

HD — only for renal injury

Standard treatment of ARDS is what we are sort of good at. ECMO I have never been impressed with, but others may hav a different opinion.

heart lung machine could directly oxygenate the blood to give time for the lungs to heal.... Code icing the patients to reduce their metabolic demands might help like they do with stemi mi’s and certain strokes...high frequency ultrasound with regular percussion and vibration might break up the consoldations in the lungs.
