Strange that there was only one hole. If it was a slug rather than bird shot or buckshot, then it seems pretty small for a slug.

The reason it is a small hole, is because it didn’t have time to spread. The gun was on his foot, and when first leaving the cartridge, there is no spread to the pellets.
Had to have been a slug. Mayhap a 20-ga. or even a .410. I agree that a 12-ga. slug would put a bigger hole through flesh, especially if it was anything like Remington’s Core-Lokt partition-nose slugs.

After that kind of damage I’d imagine his big toe would need to be amputated. Bone is exposed and lots of flesh is missing from the bottom of the toe. Prime opportunity for infection to set in, even under the best of circumstances.
I wondered about that too. But it looks like he’s trap shooting, so it was probably bird shot.
I guess since the barrel was right on the boot, the shot didn’t spread out yet.

I was similarly lucky when my chain saw got away from me and sliced into my combat boots, which luckily stopped it cold, leaving me with only a few stitches to the big toe.
