It could be a Tuberculosis cavity or bronchiectasis or tumor that ate into a blood vessel and change of air pressure made it rupture.
Bleeding stomach ulcer or esophagus ruptured from violent vomiting also could do this.
There are few symptoms until the blood vessel bursts.
