He should absolutely pay more. Aircraft fuel consumption does not care if you are muscles or fat. Only thing which affects fuel consumption is WEIGHT. To be fair, there should be a surcharge for every pound weight over 100 lbs.

If the passenger is too big to fit in the seat without interfering with adjacent passengers, that passenger needs to pay more for a larger seat. The passenger's weight matters for purposes of weight and balance calculations for the aircraft and impact on fuel consumption.
