A large stroke trial has shown that intensive blood pressure lowering after clot removal worsens recovery, stopped early due to the significance of the findings.
Professor Craig Anderson said the rapid emergence of this effect suggested the more aggressive approach was compromising the return of blood flow to the affected area.
"Our study provides a strong indication that this increasingly common treatment strategy should now be avoided in clinical practice," he said.
Around 85 percent of strokes are ischemic strokes, caused by the loss of blood flow to an area of the brain due to a blockage in a blood vessel, leading to a loss of neurological function.
Endovascular thrombectomy is an increasingly used non-surgical treatment for ischemic stroke, in which microcatheters or thin tubes visible under X-rays are inserted into the blood clot to dissolve it.
"A potential downside of this now widely used and effective treatment is that the rapid return of blood supply to an area that has been deprived of oxygen for a while can cause tissue damage known as reperfusion injury," said Professor Anderson.
"This has resulted in a shift in medical practice towards more intensive lowering of blood pressure after clot removal to try and minimize this damage, but without evidence to support the benefits versus potential harms."
Patients in the more-intensively treated group had significantly worse scores on the scale compared to those allocated to those treated less intensively.
Compared to the less-intensive group, they had more early brain tissue deterioration and major disability at 90 days but there were no significant differences in brain bleeds, mortality, or serious adverse events.
Patients who had their blood pressure more intensively controlled also rated their quality of life as significantly worse due to limitations on their physical abilities resulting from their stroke.

Of course it made mistakes; but they were genuine mistakes and measures were taken to correct them.
when I had an ischemic stroke a year ago they deliberately kept my blood pressure high for a few days afterward. Stroke wiped out my left side but it has mostly come back.

Me, as well. My stroke was in 2013. Took me a year to walk unaided and 2 years to write. 3 years to reliably write legibly.

Covid has less than a one percent fatality rate.
Shots have less than one percent fatality rate.
So why did we spend billions and destroy our economy and freedom and general health. An absolute travesty.
It is far even less for healthy young people under age 50. I would concede mortality was high when covid first arrived, for 2 reasons. No one had immunity for that virus and old people sick with covid were shoved into nursing home where it spread like wild fire. I see no need anymore for any mandates for vaccine from the gov't.
This does suggest a possible therapy. There are now some artificial bloods that carry as much as 3x the oxygen of regular red blood cells. While trying to replace a lot of blood with it may cause problems, what about using just a bit *added* to the blood?
The trouble is that while there are a bunch of Oxygen therapeutics out there, some even passing Phase III trials, there is a strong hesitancy to giving them approval instead of leaving them in the limbo of “needs more research.”
