I'll be stripping, showering, washing clothes and disinfecting shoes, light switches, door handles, whatever I've brought in and wherever I've walked. All it takes is one viron.

Kate Middleton is said to be the only royal to go out. The elderly family members are staying home. Just Harry, well, he’s dumped his responsibilities. William is being kept safe from CV. Liz is at Windsor with Phillip and no longer making public appearances or taking visitors. The family is not hiring any new staff to keep CV away.

That only works if no one and no deliveries are let in. All it takes is one piece of contaminated mail or one non symptomatic child visiting grandma and they’ve got it again.

No way can they keep that small bubble. The first time one of that group sneaks out to see their mistress or goes to hang out at the local gathering place (NZ’s version of 3 times a day trips to Walmart), goes to work or they don’t handle an infected delivery properly, they come back and infect two households.

My anecdote of the day, had patient present who has lupus and is on plaquenil. Nasal swabs negative x3. Now intubated and tracheal aspirate sample is positive. Clearly has severe disease. Has been on 200mg bid plaquenil since 2017.

“had 6 patients so far on HCQ at baseline who came in with moderate to severe COVID-19. 3 did fine, just some O2 via NC and home in a few days, 1 needed HFNC, 2 got tubed with 1 recovering and 1 dying.
So I have no conclusions whatsoever to draw from that lol”

Abstract

Coronavirus disease 2019 (COVID-19) caused by SARS-CoV-2 infection is a new rapidly spreading infectious disease. Current guidance from the World Health Organisation (WHO) highlights asthmatics as a high-risk group for severe illness from COVID-19. Viruses are common triggers of asthma exacerbations and the current SARS-CoV-2 pandemic raises several questions regarding the optimum management strategies. Here, we discuss the contentious issue of whether the mainstay therapies systemic corticosteroids should be used in the routine management of COVID-19-associated asthma exacerbations. Recent guidance from the WHO has advised against the use of corticosteroids if COVID-19 is suspected due to concerns that these agents may impair protective innate anti-viral immune responses. This may not be appropriate in the unique case of asthma exacerbation, a syndrome associated with augmented type 2 inflammation, a disease feature that is known to directly inhibit anti-viral immunity. Corticosteroids, through their suppressive effects on type 2 inflammation, are thus likely to restore impaired anti-viral immunity in asthma and, in contrast to non-asthmatic subjects, have beneficial clinical effects in the context of SARS-CoV-2 infection
