Worked in an Airborne unit for a few years.
Cause of death:Impact.
The guys used to read a parachuting magazine, back end article every month was as listed above. The magazine look at fatal accidents and tried to use that as a safety lesson.

Nineteen people have died there since the facility opened in the early 1980s, with six of them between 2016 and 2018.Who in their right mind would go to a facility that has had 6 fatalities in 2 years? That's like playing Russian Roulette.

Very little information provided. Was the cord pulled? Was the chute tangled and unable to open? Was there a reserve chute? Was the cord pulled too late for the chute to open?

Toward the end of my "stellar" Army career I did a Reserve summer camp at Cutler Army Hospital at Ft Devens,Massachusetts.
Coming back from lunch one day I saw a chopper taking off from the front lawn of the hospital,which I thought was strange.Turns out they had just delivered a bird Colonel from the Natick Labs (an Army research complex about 20 miles away) whose still-in-development parachute didn't open.
They took him directly to the Pathology Department.
