Usually, FAA probes into such situations are limited to if the chute was packed properly and by the appropriate person. Fellow skydivers said she was experienced in the extreme sport.
Meanwhile, the skydiving center itself has a decades-long history of deaths and was investigated by federal agents earlier this year, according to KTXL.
Nineteen people have died there since the facility opened in the early 1980s, with six of them between 2016 and 2018.
After trouble in 2016, 20 center instructors were suspended and it was determined 120 more needed more training as the U.S. Parachute Association investigated.

Lodi, CA. I drive by that place every so often. It’s right next to Highway 99. And yes......it is the same Lodi sung about in that song!!!! When you jump out of an airplane, things might not go well. Is it really a shock accidents happen?

You really don’t need a parachute to skydive.
You only need a parachute to skydive twice.
That’s like the old first law of submarining: The number of surfacings has to equal the number of dives + 1.
They no longer have rip cords on the main chute. Now they pull the pilot chute out of a pocket and it takes to the airstream, opening the backpack and pulling out the deployment bag. That cured the old pilot chute hesitation problems we had with the old spring-loaded chutes.
Every planned parachute jump must have a reserve.
Many jumpers now have an automatic opener on the reserve that will fire if you pass a programmed altitude at a programmed speed. The trouble is that if you have a spinning, malfunctioning main, firing a reserve into that mess can twist up both canopies. A friend of mine was killed because of a double entanglement.

Coming back from lunch one day I saw a chopper taking off from the front lawn of the hospital,which I thought was strange.Turns out they had just delivered a bird Colonel from the Natick Labs (an Army research complex about 20 miles away) whose still-in-development parachute didn't open.
They took him directly to the Pathology Department.
I look on these events as "double-or-nothing" foolishness: No matter how many times you win, you only have to lose once.
Why would anybody jump out of a perfectly well operating aircraft?
