I was wondering the same, it’s obvious from the photos of the island that a plane can not land there? They must land in a close by island and be shuttled to the island in a boat.
Google Earthing the island:
There is no landing strip for fix-winged craft. There is a helicopter landing point on a grassed tract designated with a large “H”.
Did not look at adjacent, larger islands for fix-winged capability.

planes don’t have to land on land, if you zoom in on the map epstein island little st james is east of Puerto Rico between Charlotte amalie and the British Virgin Island just south east of Great St James Island
Could a sea plane land there? Or too far out for fuel capacity?
