Hawaii's former health director who died after a plane she was traveling in crashed into the ocean was wearing an infant life vest and the pilot didn't give a safety briefing before takeoff, according to details in a National Transportation Safety Board report.
An autopsy determined Loretta Fuddy died of an irregular heartbeat from hyperventilating after she exited the plane, which landed in choppy water off the island of Molokai. The pilot and seven other passengers on the 2013 Makani Kai Air flight survived.
Pilot Clyde Kawasaki reported to the NTSB that he heard a loud bang, followed by an immediate loss of engine power soon after the single-engine Cessna took off from Molokai, headed for Honolulu.

Golly, the 150 lbs. videographer claimed that he wasn't able to stay buoyant because of his long denim jeans and steel-toed boots! It would seem these defenders of the true faith are at some kind of loggerheads.
Me? I think maybe one just needs to be good at physics, as clearly the videographer was:
What color is the back of Jake’s seat in that image label 1?
Like at image 5. The hair goes down to the back of the seat. How much lower does it go? Middle of Jake’s back?
We can’t see how far down it goes because the seat is in the way.
But we CAN see that his black hair sticks out the bottom of his hat.
On image #5 we see the whole bottom of the hat and there is no black hair sticking out the bottom. And the reddish hair that DOES stick out there goes down no more than maybe an inch. That is NOT Jake’s hair. He had black hair sticking out from his hat, drawn from the sides into a ponytail in the middle at the back of his neck. No sign of ANY of that in image #5.
No, Butter, it goes down past the top edge of the seat.
Jake's hair is not black.
Interview intro
Key and wife; note color of his hair
Key and wife; hair is below shoulders
Key and wife; hair is below shoulders & light toward bottom
For comparison, here's the one I screengrabbed of Mr Key bailing out
Closeup view to compare to bailout pic
Black hair drawn to the middle in the back. That does not show when we see the whole bottom line of the hat.
4zoltan, using the most blurry image and ignoring the clearer ones does not make you look honest at all.
Post a clearer picture.
You've been there, done that, obviously - and now you're doing it all over again.
WHAT A REVELATION...
Don’t put them on FR then.
“By the way, everyone, please be apprised I consider my posts proprietary and copyrighted with all rights reserved”
You don’t know the first thing about copyright law if you claim that while posting to FR. What an idiot.
Those items mentioned have already previously been published with a full and proper copyright notice. Thus I was citing that work, publication and copyright (though distribution has thus far been admitted limited), lest anything think they were presently in the public domain.
I have previously published some 40 works with copyright notices. None of those have yet to be challenged.
It doesn’t quite seem productive nor correct to hurl epithets.
Give it up!
The Twilight Zone is so over!
You want me to delete all your posts then? We’re mot going to have any copyright hassles here. Posters who threaten legal action get banned and their posts nuked.
(I can’t claim to be anyone’s editor. Let me improve on that last post:)
