The image below was sent to me by a reader who went for a visit to a physician, and was told that she could not be seen unless she filled out this race and ethnicity questionnaire, and put her name on the form.
She initially refused, but they insisted that it was required or she could not see the doctor. The form does indicate that it is mandatory under Section 4302 of Obamacare.
Section 4302 is explained by HHS as follows:

Section 4302 requires the Secretary of DHHS to establish data collection standards for race, ethnicity, sex, primary language, and disability status. The law requires that, once established, these data collection standards be used, to the extent practicable, in all national population health surveys. In response to this statutory requirement, this implementation guidance outlines the new minimum data collection standards for race, ethnicity, sex, primary language and disability status for implementation in HHS, along with a description of the data standards development process, the rationale for each data standard, and instructions for their implementation.

I always put “other” for race, and, if their is a blank for it, I put “human”.
Once, at the Health Department, I put “other” when there was no blank. I am about as white as one can be, so I suppose that is why I caught one of the workers studying my face every chance she got trying to figure out, I guess, why I had not checked “caucasian”.
In this society I dare anyone to question that!
The Caroline Islands are in Micronesia.
Could be, so I might check that one along with lots of others...we all have ancestors from various countries...depending on the choices. I don’t want to get into their red tape for free stuff.

This form was obviously written by an idiot, but the intent is fairly clear.
“Ethnicity” for purposes of the government is to be defined as American, not Latino/Hispanic and a whole bunch of different Latino/Hispanic groups. I got a particular kick out of their including certain Spanish regions/provinces (Andalusia, Catalonia) and leaving out others (Estramadura, Aragon, Murcia), with no apparent rationale.
They also list Colombia, Ecuador and Chile, but not Peru. What’s up with that?
The purpose of the “race” question, OTOH, is apparently to distinguish between whites, black and a massive number of peculiar Asian and Pacific Islander groups. Why?
This massive differentiation of Asian “race” by country of origin for some extremely pbscure reason does not extent west or north of the sub-continent. Iranians, Afghans, Tibetans, Arabs, etc. are apparently demoted to simply generic “Asian.”
Madagascar is for some astonishing reason included, possibly because it was originally settled from SE Asia, a region the author appears to be obsessed with.
It would probably be possible to assemble a mental profile of the author simply by more detailed analysis of this astonishing document.

The form lists Polynesian, which is a large inclusive group, but also a bunch of Polynesias sub-groups. For some obscure reason they missed Maori and Hawaiian.
I guess they did the same with Micronesia.
Hmong is listed, but none of the multitude of other SE Asian mountain groups. Some of the groups listed I’d have to google.
I believe in the ‘keep it simple stupid’ philosophy, especially in paperwork. Another reason for small government and less red tape.

Yeah, Caroline Islands -- but I betcha 93% of patients who come into a random doctor's office in North and South Carolina don't remember that, and at least 20% will probably check the box.
This form is so incredibly stupid that I don't think even the US gov't could be responsible for providing it.
If the doctor in question came from some third-world ****hole, I would run, not walk out of the office.
No mine is American....so I will leave the Carolinian box check alone. I agree, the whole racist/nationality form is unnecessary.

But you have the options, "unknown/declined" or "other".
And then there's the obvious option of "black or African American," based on the racist (but official) one-drop rule.
If the racist bureaucrats want to challenge my response, they must prove I'm not.
I have enjoyed many minutes of chuckles receiving much junk mail addressed to the "black" me as the result of my decision to answer all government-required questionnaires related to race and/or ethnicity, as "black." No problem.
Just before I left Minnstoopid to return to Texas, I was accosted by a census worker. She insisted that I pick a race, even when seh was standing there in front of me. So I said “Hew-mon”, using the Ferengi pronunciation.
The information on that form is very rarely useful for patient care, and it is not needed to verify insurance coverage or to determine physician compensation. What is the government’s justification for forcing patients to spend the time and effort required to complete the form?
Other, they forgot to list homosapian!
