Since the king has no long form birth certificate - well, only a Kenyan one, nobody needs one!
The one with the footprint is a hospital certificate.
I tried to use that (not knowing) when I got a passport in 1998.
I had to get my original BC for the passport. Mine's from Florida and it's a full page with a raised state seal on it.

The form should be completed but it should not be signed until a passport agent requests that it be done. You also need to provide parental consent for any child under age 16. Due to the different familial situations that can exist, you can refer to the table below to determine what documents are acceptable in order to obtain a passport for your child.

Proof of citizenship
Evidence of United States citizenship can be established with an original or certified copy of a birth certificate. Certified copies can be acquired at the registrar's office of the state where your child was born. Be sure to get the "long form". With a computer-generated COLB? Really?
I had to provide an original long form BC with the raised state seal on it before I got mine.
Same here - I originally thought it was cuckoo. I still think whatever's on that birth certificate that he doesn't want people to see is embarrassing but not disqualifying, or Hillary! would have found a way to get it out there before the convention, let alone the election.
When I asked the hospital about it, they said that everything is electronic now - there are never any paper copies. All the information is collected electronically and sent electronically to the state.
We had no problems getting him a passport.

Could you show us what a Virginia COLB looks like?
Or, could you tell us what items are on the COLB?
Do you have a long form birth certificate with the doctor's name and the name of the hospital on it?
If you asked Virginia for a long form birth certificate, would Virginia give you one? Thanks.

I finally got hold of someone named Christy at the Vital Records Dept. in Hawaii.
I asked about my order online of my BC and what certificate I would actually receive. Long or short form.
She told me I would be receiving the short form COLB and they no longer gave out the “Certificate of Birth”.
I asked her at least 2 times when they stopped supplying the long form and she stated it was a new policy implemented in May of 2001.
I told her I have heard of people making requests for their long form and have received it. She said she didn't think that was possible except in cases of having a very weird name. (names with hyphens and weird non-American letters.)
Then and only then will they actually make a photo copy of the original and then send it.
But for almost everyone, they will only send out the COLB.
This means to me that they DO HAVE ORIGINALS ON FILE and could if need be get a photo copy.
I kept pushing the point that I really would like the long form so I could get for instance, a passport.
She finally gave me the phone number for a Dr. Alvin Onaka (spelling?).
I had to leave a message for him to call me back. Will call again tomorrow if I haven't received a call.
So, I guess I will be receiving a COLB . But with this I will compare and contrast it with what is on my original “Certificate of Birth”.
I don't know how to post the images of each on this site, but when I receive it, I will ask for help and post the long and short form, for everyone to see the comparison.
It seems they refer people (everyone?) who wants a long form birth to Onaka. Keep insisting that you want it. He will give it up in the end. You could drop a hint that you're willing to take Hawaii to court for your own birth certificate. They can only delay - push him.

Release of the long-form birth certificate would confirm it, that's why he can't let us see it.
We know it exists because the governor of Hawaii, among others, claims to have seen it.

You can bet that if the POTUS wanted his long form birth certificate Hawaii could dig it up in a New York Minute.
But since he doesn’t want it seen by anyone, they are going to great lengths to hide it and make it unavailable.

I did back ground investigations for my agency when they came through. A COLB would NEVER be accepted as any proof of anything. They woudl be rejected as a candidate if that was all that they could provide, even if everything else looked good.
I suspect that a candidate would be able to go back after Hawaii for not making these forms available as they would hinder them from being eligible for a job.
