Here is a list of government and civilian websites that require a long form birth certificate.
1.) Snyder County, PA requires a long form birth certificate to obtain a US passport for anyone 15 years of age or younger:
"Even with a previous passport - a LONG form birth certificate is required..."
http://www.snydercounty.org/snyder/cwp/view.asp?a=730&Q=408544
2.) In the state of Massachusetts, a long form birth certificate is required to obtain a passport:
"A long form birth certificate is required for obtaining a passport and for travel outside the country."
http://www.dighton-ma.gov/Public_Documents/DightonMA_Clerk/clerk?textPage=1
3.) Once again, the state of Massachusetts requires a long for birth certificate for "proof of citizenship":
"If you were born in the United States, then a LONG FORM birth certificate is required the long form must be certified with a raised seal (no photocopies)."
http://www.cityofmelrose.org/departments/assessing.htm
4.) In the state of Massachusetts, a long long for birth certificate is required in certain adoption cases:
"However, if grandchild is a dependent of a dependent under age 19, copy of grandchilds certified (Long Form) birth certificate is required."
http://www.townofmarshfield.org/public_documents/MarshfieldMA_HR/PCHG%20Rules.pdf
5.) In Laredo, Texas a long for birth certificate is required for passport or immigration purposes:
"The long form Birth Certificate is required for PASSPORT or IMMIGRATION purposes."
http://www.laredotexas.gov/health/Birth%20-%20Death%20Application%20Final%20English%20%20Spanish%20Revised%20January%202009.pdf
6.) Presenting a short form birth certificate to obtain health insurance at IMS Health is "not acceptable":
"Photocopy of dependent's birth certificate. Must be a complete birth certificate which includes both parents' names; short form birth certificate is not acceptable."
http://staging.dakotagrp.com/3756_IMS_enrollment_guide_web/enroll/
7.) The only type of birth certificate that is acceptable by the South Carolina Department of Health and Environmental Control is a long form birth certificate.
"The only type of birth certificate acceptable for travel is a birth certificate long form."
"A short form certification or birth card (cannot be used for travel)."
http://www.scdhec.gov/administration/vr/birth.htm
8.) And last of all, to play little league in San Dimas, California, you must present a (yep, you guessed it) long form birth certificate. An abstract, like Obama's COLB, is "not acceptable".
"Supply a certified birth record and proof of residence. Abstract birth certificate is Not acceptable."
http://www.leaguelineup.com/sdlittleleague/files/Registration%20Flyer%202008.pdf
So, if a short form birth certificate is not sufficient evidence of citizenship in any of these cases, then why would it be sufficient to be President of the United States?
Yea !
We don’t have to worry about BHO trying to play little league baseball in Las Dimas !
The problem is that Hawaii no longer issues long form birth certificates.
“Although original birth certificates (Certificates of Live Birth) are preferred for their greater detail, the State Department of Health (DOH) no longer issues Certificates of Live Birth. When a request is made for a copy of a birth certificate, the DOH issues a Certification of Live Birth.”

5.) In Laredo, Texas a long for birth certificate is required for passport or immigration purposes:
Looks like Obama will never get to live in Texas.
I don’t think Obama has a BC...I think he was born in a mud hut in Kenya with some witch doctor shaking bones over him...
Because he was elected by a wide margin. Because the constitution says NOTHING about the long form, contrary to the daily claims of birth-certificaters here.
“FREE THE LONG FORM!”
I’d like to ask Hawaiian officials why they changed the law just to help Obama cover his past.
B) The Constitution says nothing about proving natural-born status at all, so your "long form" saying here is meaningless.
C) In the absence of any constitutional specifics, a challenge to the status of an individual (for any reason) would be adjudicated via statutory law. The posts here show that for much less important and weighty purposes applicants are required to show original long form birth certificates. The real question is why this man refuses to do what any ordinary citizen does as a matter of course when asked, and provide his original long form birth certificate?-
Well, the Constitution says nothing about the long form, that’s true. This is a moral argument. Does anyone have a moral right to hold the highest office in the land while concealing about 20 kinds of vital records, and fighting efforts in court to reveal them? (Especially in a case where one parent was a foreign citizen and international travel prior to the birth was not unlikely.) If no court compels it, I hope that at least by 2012, several states will have passed laws requiring documentation of natural-born citizen status with a long-form birth certificate.
In New Mexico, a short form birth certificate is NOT acceptable for a Concealed Carry Permit. ONLY the long (real) birth certificate form will be accepted. However, in the case of having the “button” to nuclear weapons, apparently the federal government doesn’t require as much as New Mexico.
The Constitution was written by honorable men for honorable men. It is dishonorable to conceal your past.
There are so many I cant count the number of birth-certificaters that throw that “I am defending the constitution and don't care about anything else” crap. At least you are not trying that.
I think you got a pretty crappy moral argument. The first AA president (elected by a wide margin) and the moral argument is he shouldn't be president because he hasn't shown you his long form BC??? Not policy, not actions, not elections but his BC. Moral??
This is why no one will touch this birth-certificater stuff. This is poison
What do you think of someone who tries to conceal his past? Do you want to trust him?
Please post the answer that you get here!!!
The usurper calls the framers “The So-Called Founders”!!!
Ping
What do you mean past? You mean his birth? You really think anyone except a few ..I wont say.. are going to hold his birth against him? Especially being the first AA American??? Suppose his father's name is not on it. Worse here because you don't even have that dirt on him. You are fishing for stuff no one wants to know. This is poison.
