Originally an unconverted member of a people, who does not acknowledge the God of Judeo-Christian revelation. Biblical translations fluctuate between "heathen" and "pagan" for unbelievers, e.g., Psalm 2:8. The term has come to mean anyone who does not believe in God, and less commonly those who are believers but are not Christian or Jews.

One who does not believe. Formerly applied to all non-Christians, the term is now used only of unbelievers, i.e., professed atheists or agnostics. (Etym. Latin in-, not + fidelis, true, faithful: infidelis, one that cannot be relied upon, faithless.)
In the Old Days, there wasn't really such a category: people either were theists of some kind, or they kept quiet and went through the motions.
"Pagan" implies belief in a god or gods other than the True God, so it wouldn't be synonymous with "heathen" if "heathen" includes atheism.
