Update: United Airlines confirm inspections have found loose bolts on the 737 MAX 9
“Since we began preliminary inspections on Saturday, we have found instances that appear to relate to installation issues in the door plug – for example, bolts that needed additional tightening.”
Assuming that pic is legit, doesn’t the washer look undersized for the hole let alone the nut not tightened down?
Those look like lock-washers, but they should have a flat washer behind them.
Yup. They built a big posh regional HQ just south of Seattle where a horse race track once stood. That building is now vacant if I recall.

I would say that inexperienced employees were installing the door. I have removed and reinstalled countless panels and never had one fall off.

These doors have nothing to do with NC. They are built and installed by Spirit in Wichita and shipped semi rigged to Renton. The doors are removed in Renton for interior installation and then rigged for final delivery in Renton.

According to several fairly authoritative remarks I’ve read that’s not the case.
I often use star lock washers — either internal or external — when I’ve got them but this is just hack work on dirt bikes mostly.
I always liked the nyloc “aircraft nuts” when I was working on dirt bikes. This was a *long* time ago.
