Clotting for any reason is a problem with any person his age.
Clots just don’t “happen.” There is a context. That is what we are not getting. (I don’t really care)...but, most people just think Clots are a way of life for some people.

Good luck. My prayers are with you. I had most of my ascending colon removed on April 30, 2015 due to Stage 2 colon cancer that was originally detected by a routine blood test that showed low iron. Since then I’ve had 2 colonoscopies and 2 CT scans. So far, so good and a favorable prognosis. Right now I’m an old fart with only 2/3 of a farter.
Maybe it was minimally because they suspected the answer before drilling? No surgery is without risk either. Just the administering of anesthesia can cause problems not to mention what you can “catch” while in the hospital.
Regarding him, why back to Az? Is not John's Hopkins and Bethesda Naval Hospital close by DC? This has the sounds of not coming back any time soon to me.

Go back to the news releases about Teddy Kennedy's final illness. It was exactly the same way. One of the first articles, from the NYT, appeared to be inconclusively optimistic except for a link to a medical article that gave the full story about his type of terminal cancer. AP articles at that time about the same matter all appeared optimistic, except for the crucial omission of details that would tell the truth.

Those are frozen sections, and they are a bit less reliable than standard histology. Also, if you are going to look for specific markers, that require using antibodies, it takes awhile. That said, this sounds like just a localized subdural hematoma, possibly caused by hitting his head at some point that he doesn't remember.
