All *presidential* motorcades have an ambulance, and seemingly those of first ladies too. AFAIK, the ambulance is always from the local area. Do Hillary’s ambulances bear local logos?
It’s not “SOP” for all Secret Service protectees to be followed by an ambulance, else Trump motorcades would have them as well.
A good guess would be that Her Heinous wants the peasants to view her as President-in-Waiting, and insists on a presidential ambulance. Something else that has backfired on Ol’ Hill.

I personally saw Trump's motorcade yesterday in NYC. I was just inches away and filmed the whole thing. No ambulance.

One supposed explanation is that former first ladies get a higher level of protection than mere candidates.
I can't really say I've looked. But I would assume that Trump would be entitled to the same level of protection that any other candidate is. And that include medical emergencies. There are a lot of crazies out there.
