Heres a bold prediction for the new year. By 2020, the American health insurance industry will be extinct. Insurance companies will be replaced by accountable care organizations groups of doctors, hospitals and other health care providers who come together to provide the full range of medical care for patients. Already, most insurance companies barely function as insurers. Most non-elderly Americans or 60 percent of Americans with employer-provided health insurance work for companies that are self-insured. In these cases it is the employer, not the insurance company, that assumes most of the risk of paying for the medical care of employees and their families. All that insurance companies do is process billing claims.
For individuals and small businesses, health insurance companies usually do provide insurance; they take a premium and assume financial responsibility for paying the bills. But the amount of risk sharing that is accomplished is limited because the insurers charge premiums that vary, depending on the health of an individual or a group of employees, and use their data and market power to identify healthy people to cover and unhealthy people to exclude from coverage. (The health care laws total ban on exclusions for pre-existing conditions will begin in 2014.)

That particular system should have been killed a long time ago. It’s just that it’s replacement ain’t so great.
Also, starting in 2014 the sick will be allowed to die instead of expensive procedures. In other words, if you have a stroke, the one shot that can save your life, cannot be given. Even today, if you do not follow your doctors exact instructions, and you are on medicare, the government can cut the payments to the doctor. This means that the doctor can drop you and you will play hell finding another one. This will happen even if you disagree with your doctor.
I see tons of help wanted ads in the health care profession around here. I want no part of them.

Once the totalitarians redefine "insurance" to include massive welfare shifts, definitions become meaningless. Of course actual insurance providers can't possibly compete with the welfare component, which multiplies liabilities at least twofold.

As in most Euro nations, the health insurances (for the most part) are actually for a nationalized system since this “mandates” people to buy insurance. National health care = forced purchase of a product. What you really have is a sort of collusion (albeit completely legal in this case) between the government and industry. Another development is that the legal recourse gets limited in such a government run system. The government through it's panels, committees, obscure bureaucrats with no name and legally top covered processes becomes the shield off which any legal recourse becomes futile. The government sets “standards of care,” and as long as those are followed there is no recourse even if a new procedure or medication is available, even if the standard goes against the moral compass of the patient or you have an exception where a doctor would recommend a deviation in the standard of care (example: Medicare, Medicaid, the VA system). Doctors in a national health care system don't practice health care, they essentially become no more than a flow chart administering care that is already pre determined based on age, sex, the type and the advance of a certain ailment. Even competition goes away and as in Germany (TK, AOK etc) all your insurances are simply different in name only but essentially the same. The government sets the standards and the differences between insurance providers are minuscule. You no longer have as much of the competitive pressures as in the past. Insurances in this sort of system actually do quite well.
