Ministry of Health has not established the exact diagnosis of the epidemic disease in the western regions of Ukraine. Health Minister Vasyl Knyazevich has given information about spread of diseases in the Ternopil, Ivano-Frankivsk and Lviv Regions today at the meeting of Cabinet of Ministers.

According to the Minister, the World Health Organization is ready to render assistance to Ukrainian experts and the Ministry of Health in order to establish the cause of death and development of disease flu in the Ternopil, Ivano-Frankivsk and Lviv Regions.

"We are working at receiving the vaccine against influenza A (H1N1) also", - said Vasily Knyazevich and asked the Ministry of Foreign Affairs to render assistance.

Answering question of Prime Minister Yulia Tymoshenko about the definition of the specific nature of the disease, the head of the Ministry of Health said: "The nature of infections is viral one. But at the moment it is not defined, is it the Californian influenza H1N1, or it is our seasonal influenza. To date, there is no precise diagnosis."

According to the Minister of Health, to date, 12 adults have died in the Ternopil Region. "There are problems, people of working age 20-45 years are sick, there is a problem with pregnant women (we have the death in the Ivano-Frankivsk and Ternopil Regions), 17 patients remain in the grave condition, 4 of them have artificial pulmonary ventilation", - said Vasily Knyazevych.

EU must help Ukraine to fight flu - Poland

Polish Prime Minister Donald Tusk has called on the European Union to help neighbouring Ukraine fight a flu epidemic and a growing number of H1N1 cases, saying its further spread is threatening the wider bloc.

Ukraine shut schools, banned public meetings and restricted travel earlier this month in an attempt to combat the H1N1 flu epidemic, which has also caused public concern in Poland.

“The character of this threat demands that rapid action be undertaken at the European Union level,” Tusk wrote in a letter to European Commission President Jose Manuel Barroso and Swedish Prime Minister Fredrik Reinfeldt, who holds the rotating EU presidency.

“This will serve both the interests of Ukraine, and the EU itself,” he said in the letter, a copy of which was obtained by Reuters on Tuesday.

Tusk urged Brussels to hold an extraordinary meeting of the EU’s health council to identify Ukraine’s needs and discuss availability of vaccines.

Poland’s Health Minister Ewa Kopacz said there had so far been 185 cases of swine flu and around 41,000 of seasonal flu diagnosed in the biggest ex-communist EU member. There have been no deaths.

“I don’t understand why we are all so worried about a rare flu that has a much more gentle course than the regular one. I don’t understand why people do not take vaccines for seasonal flu,” she told a Polish television channel.

But news broadcasts showed Poles lining up at pharmacies to get sanitary masks which were quickly sold out in a sign of growing public concern.

Deputy Foreign Minister Andrzej Kremer told Reuters that Warsaw was awaiting a reply from Brussels, and said he feared the epidemic could spread rapidly in eastern Europe.

“Poland has already organised quite reasonable help for Ukraine, but in a split second we will see swine flu in Belarus, we are already hearing something about Romania,” he said.

Shipments of medicine and medical equipment have been leaving Poland for Ukraine in recent days. A growing number of swine flu cases have also been reported in Germany and Slovakia, which likewise share borders with Poland. (Writing by Gabriela Baczynska; editing by David Stamp)

Work will initially begin in Lviv region, where reported numbers of cases showing severe manifestations of acute respiratory illness have been especially high. Two virologists on the team have started working at the National Influenza Centre and the laboratories of the Central Sanitary and Epidemiological Station in Kyiv to provide diagnostic support.

The above comments from the WHO update on Ukraine strike a more serious tone than yesterday’s quotes from WHO spokespersons playing down the alarming number of hospitalized and fatal cases, as well as media reports spinning political considerations, or politicians claiming only 15 confirmed H1N1 cases. As the WHO update clearly states, the dominant influenza circulating worldwide is swine H1N1 and it is assumed that most cases in the Ukraine are swine H1N1. Since the official government website lists 19,189 influenza cases (in addition to more than 235,000 ARI cases), the willingness of politicians to cite 15 confirmed cases, as well as media support in the distribution of the propaganda, is unfortunate.

The more severe manifestation of ARI cases are clearly hemorrhagic disease that fills lungs with blood and produces bleeding at all orifices, which are stark reminders of 1918 pandemic cases which were also linked to a swine H1N1.

Samples have arrived at Mill Hill in London, and sequence data should be available shortly. Since current swine H1N1 produced lethal infections in previously healthy young adults, and produced hemorrhagic disease, changes in the viral genome may be absent or minor. Therefore, release of the sequences to the entire scientific community would be useful.

Similarly, an update on the number of hemorrhagic cases would be useful. Rumors have suggested that such the number of such cases is significantly higher than the six described in media reports.

WHO ‘assumes’ Ukraine gripped by swine flu

GENEVA — The World Health Organisation said Tuesday that it was valid to assume that most of the cases of influenza reportedly sweeping through Ukraine were caused by the pandemic A(H1N1) virus.

Ukrainian authorities on Sunday launched an urgent appeal for help from world powers after it imposed drastic measures to tackle a sudden surge of flu-like illness in the country.

More than 250,000 cases have been reported, according to official Ukrainian figures cited by the WHO.

“Given the potential significance of this outbreak as an early warning signal, WHO commends the government of Ukraine for its transparent reporting and open sharing of samples,” the UN health agency said in a statement.

“Laboratory testing in Ukraine has confirmed pandemic H1N1 influenza virus in samples taken from patients in two of the most affected regions,” it added.

“As the pandemic virus has rapidly become the dominant influenza strain worldwide, it can be assumed that most cases of influenza in Ukraine are caused by the H1N1 virus.”

The highest rates of “acute respiratory illness/influenza-like illness” were found in western areas, while they were also increasing rapidly around the capital Kiev, according to the WHO.

Seventy-one people have died as a result of flu and acute respiratory infections, the Ukrainian health ministry said, but the pandemic A(H1N1) virus has been confirmed in only one of the fatal cases.

The WHO said a team of specialists who arrived in the country on Monday will initially focus on the Lviv region, where reported “severe” cases of acute respiratory illness “have been especially high.”

Samples from Ukraine have also been received by the WHO’s influenza research laboratory in London for tests.

“Many questions remain to be answered,” the WHO said.

“The outbreak in Ukraine may be indicative of how the virus can behave in the northern hemisphere during the winter season, particularly in health care settings typically found in eastern Europe,” it added.

Local analysts have suggested that rival Ukrainian leaders may be trying to exploit popular fears in campaigning ahead of January 17 presidential polls, as each candidate seeks to take the initiative in fighting the outbreak.

Secret document: Belarusians die from pneumonia complicated by swine flu

Medical workers report to the Health Care Ministry that, about ten people have died in Minsk from pneumonia, a A(H1N1) flu complication.

Journalists managed to get a medical report saying the cause of death of two persons of 27 and 36 years old (they died on the same day) was pneumonia. RNA of A(H1N1) flu were detected in their tissue samples.

The 27-year old man had a high temperature and went to a local polyclinic, was treated at home, but after some days taken to hospital #6, where he died on October 26.

According to a source of “Belorusskie Novosti”, it is mostly young people who die from the A(H1N1) virus in Belarus.

The difficulty is that a death certificate is only the first step to prove the cause of death was the virus. According to the source, it give the officials an opportunity to sit on their hands and speak about the mixed infection, etc. Post-mortem and histologic examinations are needed to confirm the death was caused by the A(H1N1) virus. Medical officials can state the flu is just a background disease, but not a ground for the complications, and will be right formally.

For the moment, the medical worker has information about ten confirmed death cases of A(H1N1) flu. He states: “The pneumonia, we are speaking about, is a complication of A(H1N1) flu, it is not a bacterial pneumonia. A sick person chokes and can’t breathe. He prescribed artificial ventilation, but dies. This takes three or four days.”

The source notes a fast growth of pneumonia cases in Belarus and in Minsk. He states that the number of fatal cases exceeds the normal statistics for this period of year. There are many patients with pneumonia in intensive therapy units. He can’t say whether all of them are checked for the A(H1N1) virus.

The physician’s statement can’t be proven officially. According to some reports, medical workers are not permitted to say any information about the current situation with the A(H1N1) virus. Requests to the Health Care Ministry remain unreplied. The ministry representatives deny death cases of swine flu or refuse to give any comments in order to prevent panic. Officials never think it is impossible to hide the truth for long, it will leak and gather rumours and make even greater panic.

Attempts of the authorities to hush up the situation are short-sighted, the physician said. The current situation needs not reports saying it is under control, but urgent measures.

He gives an example of behaviour of the Ukrainian government: “The country has asked for help and received it. 600,000 doses of Tamiflu, bought by the Ukrainian government, had already been delivered. In accordance with medical norms, this is enough to supply all regions of Ukraine and make reserves.”

The medical worker emphases that Belarus becomes a proving ground where the virus is tested in natural condition, when preventive measures are insufficient and treatment is minimal: “The duty of the health system is to buy the necessary medicines beforehand and be able to forecast keen demand. At least, sufficient number of protective masks must have been purchased. It finds out that our medical system is not effective to fight the epidemic. The officials continue to state the virus is raging in Ukraine and Russia, but not in Belarus. “

The physician emphasized: the Belarusian medical workers are not allowed to give information to the public (that’s why the source wants to stay unanimous). He proves this with own examples: “We are constantly told not to sow panic, which means we can’t give true information. We are strictly forbidden to speak with the press... An attempt to decrease the number of disease cases in Belarus comes to absurdity and forgery. Medical issues become political issues. Ministry of Health confirmed 80 cases of death from pneumonia, said deputy chief sanitary doctor Ludmila Muharskaya.

“The latest figures have died from pneumonia, not from the flu and colds, and pneumonia 80 people”, - said, in particular, she said.

At the same time, as reported by Channel 5, according to local authorities, the number of deaths from the flu epidemic in Ukraine has reached 86 people.

It reported, recorded 2 other fatal cases in the Lviv region, all in the area - 39 deaths. In the Volyn region, local authorities ascertain 3 more deaths.

Later, the authorities reported that in the Lviv region the number of deaths from influenza and ARI rose to 37 people, and Lviv Mayor Andriy Sadovaya asked the presidential candidates called for transfer of funds from the election funds for the acquisition of modern virology and microbiology laboratories.

Meanwhile, Prime Minister Yulia Tymoshenko, speaking in the Rada, declared stabilize the epidemic in the eight western areas.

10 hours the number of patients in the Carpathian region increased by 6 thousand

Today, 3 November, in the Carpathian region of influenza and ARI patients increased by 6 thousand people.

This was reported in the management of information and public relations of the Transcarpathian Regional State Administration.

As stated in the message, according to the Health Authority, as of 18:00 Tuesday, November 3, in the Transcarpathian region registered 18 thousand 618 patients with acute respiratory infection and influenza.

Number of patients at 8:00 am on Nov. 3 amounted to 12 thousand 805 people. That is, in fact 10 hours the number of patients increased by almost 6 thousand people.

Of the total number of patients already hospitalized in Transcarpathia 851 people. Among the sick - 4 thousand 222 preschool children, 4 thousand 728 - school age and 9 thousand 668 adults.

Clusters of Hemorrhagic H1N1 Pneumonia in Ukraine

All the six dead young people had symptoms of severe hemmorhagic pneumonia. The disease starts slowly, with temperature rising to 37.2 - 37.3 degrees, slight cough and pain in joints. Nasal catarrh developed at the end of the second or third day. Autopsy revealed that the lungs were soaked with blood, the oblast chief specialist said.

The above comments are from an early story describing cases in Ukraine. The clustering of hemorrhagic cases raised concerns. The concern was increased by anecdotal reports citing a high frequency of such cases in Lviv (see map). The recent WHO announcement that they were initially focusing of severe acute respiratory illness in Lviv also increased concerns.

Hemorrhagic pneumonia was also observed in the 1918 pandemic and was thought to be linked to cytokine storm. Consequently, those with robust immune systems (previously health young adults) disproportionately died, which has also been seen in the current outbreak (Mexico, US, and worldwide). However, the cases in Ukraine appear to be clustered, raising concerns that the virus has changed.

However, the change may be minor, since the current H1N1 has produced the above symptoms in earlier cases. More severe cases may be linked to a higher viral load, which could be linked to minor genetic changes, or simply due to concentration of virus in schools. In the US the spread of the virus has been linked to school openings, which lead to an explosion of cases and subsequent student deaths. However, now there are increases in fatalities in teachers and administrators, increasing concerns worldwide.

In Ukraine, weather changes and heating issues may have led to a surge in cases, and increased concentrations of virus could have produced conditions favoring high viral loads and increased cytokine storms.

Samples have been sent to Mill Hill, and sequence data should be released shortly. Similarly, investigations on hemorrhagic pneumonia in lviv should help resolve the mechanism for the high concentration of such cases in western Ukraine.

However, H1N1 spread in Kiev has been noted, and other countries such as Belarus have announced additional H1N1 suspect or confirmed fatalities.

Sequence analysis of these cases and an update on the frequency of hemorrhagic pneumonia in Ukraine and worldwide would be useful.

Ukrainian News Agency “Fraza” reported that, according to informed sources, “it has been confirmed 100 % Pneumonic Plague in Ukraine”.

The Agency asserts that “the head physician of the medical institutions has sent out an informal disposal - not to sow panic, to refute the information about the plague, and to speak only of swine influenza”.

It is also required to distribute masks at health facilities with 8 levels of protection and anti-plague protection costumes. There is also an informal order not to allow any visitors to see the patients.

According to the “Fraza” agency, “today in Ukraine pneumonic plague is going in parallel with swine flu. The plague has killed over 60 people, and about 14 from the flu.

“The situation is complicated and unpredictable. We found a strain of this virus in our laboratory ... This virus is dangerous because it can mutate, and we can get a very different kind of virus, which was in Latin America or in Mexico, now it is European “, - said Lazoryshynets at a press conference, the “Correspondent” magazine informs.

At a press conference Lazoryshynets also said that there are 22 officially confirmed cases of influenza A/H1N1 in Ukraine.

First Vice Prime Minister Oleksandr Turchinov previously stated that as of Monday morning, November 2, there are 64 registered deaths in Ukraine from flu and acute respiratory viral infections.

Meantime, according to official information the highest level of epidemiological risk is declared in Ivano-Frankivsk (Stanislav) area, from 2 November. This decision was made by Ivano-Frankivsk Regional emergency epidemiological commission at the extended session on November 2.

In accordance with the Commission, quarantine will last for all educational institutions in the area until 20 November, a commission to monitor the distribution of medicines is also made, which come in the region as humanitarian aid.

The commission agreed that health workers will provide individual first aid kit. There will be extra transport allocated to visit patients by the district doctors.

According to the Ministry of Health, today in Ukraine there are 255 000 registered cases of influenza and SARS, of whom 15 000 were hospitalized.

Ministry of Transport and Communications of Ukraine introduced a permanent monitoring of the situation on a clear implementation of the planned anti-epidemic measures in public transport.

A special monitoring group studied the transport department on November 2, how the anti-epidemiological measures were applied at the Kiev railway station, central bus station of the capital and the International airport “Borispol”.

Polish epidemiologists November 3 arrive in Ukraine for obtaining specimens from patients with the Ukrainians. The experts will arrive in Lviv, where it is supposed to take 190 samples.

Timoshenko: Ukraine’s 10 regions close to the epidemic threshold

Prime Minister Yulia Tymoshenko said that in 10 regions of Ukraine the incidence of influenza is approaching epidemic threshold.

According to an UNIAN correspondent, Yulia Timoshenko stated this today at selector meeting with heads of regions to prevent the spread of infectious diseases.

“Areas that are close to the epidemic threshold, and where necessary to implement all control measures - is Vinnytsia Dnipropetrovsk, Donetsk, Zaporizhzhya, Kyiv, Luhansk, Odesa, Poltava, Sumy, Chernihiv”, - said Tymoshenko.

According to her, today’s session the Government should consider introducing quarantine measures in these areas, including - a ban on holding public events, the cessation of educational processes in schools and institutions.

Y. Tymoshenko reminded that in 9 regions still exceeding the epidemic threshold. In particular, in the Volyn, Transcarpathian, Ivano-Frankivsk, Lviv, Rivne, Ternopil, Khmelnytsky, Chernivtsi regions and the city of Kiev.

The Prime Minister also added that in the ARC, Zhytomyr, Kirovograd, Mykolayiv, Kharkiv, Kherson, Cherkasy regions and in the city of Sevastopol remains “relatively prosperous epidsituatsiya. “In these areas there is no threat of approaching epidemic threshold”, - said Tymoshenko.

In Kiev - the first death from influenza

Prime Minister Yulia Tymoshenko said that in Kiev, one man died from influenza A/H1N1.

According to her, she had just received confirmation of death from swine flu of the Minister for Health Vasyl Kniazevych.

First Deputy Chairman of Kyiv City State Administration Irena Kilchytska reported that the woman died yesterday from pneumonia.

“Today is the first confirmation of suspicions that the California flu,” - noted Kilchytska. She also reported that in Kiev, influenza and acute respiratory viral infections suffer from 26 thousand people, almost half of them - children. The epidemiological threshold in Kiev exceeded by 14%.
