The Shahab 3, 4 and more recently Shahab 5 solid fuel missiles at the site are hidden in old salt mines in the mountain face... Sounds like perhaps a considerable amount of energy was released in a confined space.
As to how that might have come to happen, well... I suppose we'll never know.

Well, there are all sorts of work accidents possible. The automotive airbag propellant factories have had troubles over the years along with numerous fireworks factories.
On the other hand, it's not a good idea to go around pissing folks off.

There is no evidence that the Shahab-5 has ever even been flight tested. I strongly doubt that the Iranians are close to making a solid fueled missile. Solid fueled IRBM are a vastly more difficult to make (and tactically more effective) than liquid fueled.
I know that the Iranians claimed to have made a solid fuel IRBM of some sort, they say a lot of things I'm not buying.
