The photographers assistant could be holding a reflective panel to get the light right.

No he hasn't been pasted in. You can see from the sunlight hitting the bystanders heads, the gunman's head and forarm and gun barrel and stock, that the light source is the same.
Someone else also mentioned the focus of all involved; the person speaking of that is apparently not familiar with depth of field capabilities and settings of high end camera equipment.
I don't believe it's been "shopped". Staged perhaps, but not shopped.
