There will be a thorough investigation by BP, OSHA, the EPA and other agencies. It will take some time, but the bottom line is that somebody didn't tighten up something the way they should have.

While it could have been a terrorist attack - I think we would have seen a different mobilization of police. Just a guess.

I don't think that there is any reason to be suspicious in this case. Of all of the units in the refinery, damage to this one will have one of the least impacts on the refinery's ability to produce oil products. If a terrorist who knew what he was doing was capable of taking out a unit, he'd go for some other unit. I'll avoid specifying which ones would be first on my list, but the notion that there's any point in not stating them is silly. Everyone with any knowledge of refining knows which units to hit to do the most to cripple a refinery.
As I said in my previous post, there's plenty of reason to believe that this isn't a terrorist attack. Of all the units in a refinery, damaging this one causes one of the least disruptions. I'm not saying that there won't be problems, but if they really wanted to cripple a refinery, they'd go after one or more of several other units. If they were capable of doing this kind of sabotage, they wouldn't have hit this unit.
Actually, the governmental agency that does much of the root cause investigation these days is called the U.S. Chemical Hazard and Safety Board. They are on the web at www.csb.gov and have a pretty good site. I try to read all of the reports that they do on these incidents because my work involves trying to mitigate problems several steps before they reach this point. I'm still not certain that I believe that the government should have this agency and be doing this kind of investigation. However, as long as our tax dollars are being spent this way, I'm going to get all the benefit that I can.

No, it has not been forgotten. The electric utility here is spending a LARGE amount of money, relative to previous years, inspecting and strengthening the electric transmission system.

I believe the last time I saw that written was in the book, 'The Mote In God's Eye' by Niven and Pournelle.
IMO, 800 wasn't claimed because of Al Qaida's bigger plans and they didn't want attention brought to the airline industry pre 9/11.
