Female Black Hawk Pilot Who Crashed Into Airliner Made Multiple Mistakes: Report

The Black Hawk was 15 seconds away from crossing paths with the jet. Warrant Officer Eaves then turned his attention to Captain Lobach. He told her he believed that air traffic control wanted them to turn left, toward the east river bank,” the Times reported.

“Turning left would have opened up more space between the helicopter and Flight 5342, which was heading for Runway 33 at an altitude of roughly 300 feet. She did not turn left,” the report stated.

2nd mistake ~ Her dereliction killed over 60 people.

That was the 3rd mistake.

The second mistake was not being immediately relieved of control by the Pilot in Command who should've then taken control of the helo.

First mistake in my opinion is, in the rush to get mandatory flight time for return to an aviation unit, she allowed the tappings of the White House to not retain current training.

This is a classic case of privilege leading to one’s death... what did she know, or just what was she actually doing... some flight history shows a prior possible run at another airliner.

But you are right.

Even if a flight student of mine is a four-star general and he freezes at the controls, causing a dangerous situation, you needn’t ask me whether I’d punch his lights out and take control of the aircraft before I let him kill everyone in the plane.

If I didn’t believe I could overpower each of my students, I’d never fly with that person. But yes, I do generally keep that fact to myself.

WO Eaves was as qualified an instructor pilot as he was ever going to get for the Blackhawk flight that day. We may be darned sure knew he was authorized to take over control of the helo.

He hesitated and was lost.

They were training for continuity of government evacuations. It is likely if such a scenario played out there would be even more stress (think 9/11 evac’ing the Pentagon). In the thick of it is not the time to find out your pilots can’t handle stress or fly the prescribed route.

“Train how you fight,” is a common military motto. Unfortunately, training can be just as dangerous as “real world” scenarios, and in this situation 60+ civilians were lost. Worse - this highlights the fatal consequences of DEI. Worst - the side effect will likely be watering down training, which will have far broader impacts.

The helo crew caused this, the jet pilots were the only pros that night.

This flight route should have never been approved for training let alone night vision training.

Oh bs, it was night VFR, the crew had NVGs on, they also had over a hundred feet over the ceiling for this route. Classed airspace is just like uncontrolled airspace it just has requirements. These weren't followed.

I thought the instruction from the Tower to the helicopter was vague and irresposible

ATC wasn't doing the flying, the last 12-20 secs were in the vid, VFR means see and avoid. No way they couldn't see the jet with ALL its lights on.

Unfortunately the negligence occurred years before this incident. This flight route should have never been approved for training let alone night vision training. To do so was asking for an incident such as this, it was also insane IMHO.

They need to do something about DCA. Perhaps close it down as a civilian airport. Or ban the military from using the airport.

We have Dulles and Baltimore. So we really don't need DCA any longer.

The warrant officer couldn’t take controls from her, even if he had more flight hours. She was a commissioned officer.

She was the copilot, there is no rank in the cockpit, just Aircraft Commander and copilot. Only person that can take over from the guy who signed for the AC is a General in his chain of command. Since they are a general they probably didn't get there by being stupid.

It doesn’t matter. I think the ATC order was mere seconds before the crash. Aircraft in this airspace are routinely allowed to close at much lower limits than by statute. Just insane in a congested commercial airspace with large numbers of passengers transiting daily.

Seems like everyone is going to have to boycott DCA until they find a way to fix it. It may not be fixable at this point.
