The Black Hawk was 15 seconds away from crossing paths with the jet. Warrant Officer Eaves then turned his attention to Captain Lobach. He told her he believed that air traffic control wanted them to turn left, toward the east river bank,” the Times reported.

“Turning left would have opened up more space between the helicopter and Flight 5342, which was heading for Runway 33 at an altitude of roughly 300 feet. She did not turn left,” the report stated.

That the chopper pilot made multiple mistakes was obvious to everyone the moment they saw it happen. But sure, an official government report might help it from happening again. If the actual collision didn’t do that already. Now the FAA will have new directives and policies. Which also should have been obvious before this happened.

What was he doing before he turned his attention to her? "His attention" presumably would not have involved turning his head to look at the big fat helmet on her head from the side. (Such would only have been a further waste of precious time.)

He should have been commanding, "My control!" or whatever their agree-upon verbiage to that effect was.

WO Evans had been working the radio (with the controller), so he should have been visually scanning for the CRJ. His awareness that Capt. Lobach hadn't turned left nor descended to be within the limits of Route 1 should have already been at the top of this thinking.

I find it difficult to imagine there was an redirection of his attention, as if he looked up from where he'd fictionally dropped a picture of his wife (Sum of All Fears reference).

She was not aviating (without mistakes). Unless Evans hadn't noticed or didn't care--neither of which I would believe--WO Eaves' attention should have been 100% on making sure the craft was where it was supposed to be altitude-wise and including performing the controller's implicitly-instructed left turn toward the east bank.

Previous reports were that Capt. Lobach had 450 hours of experience, not 45. If that's accurate, someone's apparently been engaged in cover-up, which would suggest mens rea, perhaps where the Biden connection might indicate proper, high standards had already been subverted.

She was pilot in command. The warrant officer couldn’t take controls from her, even if he had more flight hours. She was a commissioned officer.

Captain Rebecca Lobach, 28, was the pilot flying and undergoing her annual night flying evaluation. She had accumulated 450 flight hours at the time of the crash, with 326 on UH-60 type helicopters.[20][21][22][7]: 3

Chief Warrant Officer 2 Andrew Eaves, 39, was the evaluator for Lobach and was serving as the pilot monitoring who primarily communicated with air traffic controllers. He had 968 flight hours, with 300 on the UH-60.[23][24][25][7]: 3

Staff Sergeant Ryan O’Hara, 29, was the crew chief (aircraft maintenance technician), with 1,149 flight hours, all being on the UH-60.[26][13][7]: 3

Well, when people are raised to believe they can do anything, they lose all sense of what's possible, given their capabilities. Also, and more importantly, they lose the measure natural humility that all rational, mature people possess.

I’m not a pilot, on the net or IRL, but I’d think if you are the pilot using your instruments and wearing NVG (?) she should have quickly taken directions from the tower, ‘turn left’.
The WO didn’t have time to radio a major, “Would you tell this ...
