Diabetes drug linked to increased risk of severe eye condition (GLP-1RAs / semaglutide)

Two independent studies show that patients with type 2 diabetes who are treated with the drug Ozempic have an increased risk of developing damage to the optic nerve of the eye, which can lead to severe and permanent loss of vision.

Both of the new register-based studies from SDU confirm a concern that was raised earlier this year in a smaller American study.

"We have examined data from all 424,152 Danes with type 2 diabetes. We found that Ozempic more than doubles the risk of developing NAION. NAION is damage to the optic nerve of the eye caused by a sudden stop of blood flow to the optic nerve,," says Grauslund.

In Grauslund's study, the researchers divided the patients into two groups: those who were treated with Ozempic and those who received other treatment.

Anton Pottegård and his research group, who are experts on medicine use, have closely examined a smaller but more specific sample of the group of patients with type 2 diabetes. They have compared new users of Ozempic with users of another drug used in the treatment of diabetes who are at the same stage of the disease.

As with the American study, both studies show that Ozempic increases the risk of developing NAION and that this applies to everyone with type 2 diabetes and not just to those patients who are already very ill.

"The American study proposes a hypothesis about a possible association, so it was important to investigate," explains Pottegård.

"However, treatment with Ozempic should be stopped if NAION is detected in one eye. The vast majority of patients can be confident in receiving treatment with Ozempic, as the absolute risk is so low, but some people may prefer an alternative treatment following consultation with their doctor."

From the study abstract, Ozempic (also known as semaglutide, a glucagon-like peptide-1 receptor agonist (GLP-1RA)) had a hazard ratio of 2.81 versus sodium-glucose co-transporter 2 inhibitors (SGLT-2is) like Jardiance, Farxiga, or Brenzavvy.

The absolute risk was still quite low for Ozempic-type drugs, with an apparent increase of 1.41 people affected over 10,000 use years.

Do note there appears to be a likely reason for why Ozempic-type drugs are more inclined to do this, so keep this in mind when looking to use these weight loss drugs.

I would expect that it also causes hypoxia and drusen to form, resulting in macular degeneration.

Thanks, a relative of mine is on Trulicity (for Type 2) and I never see that drug listed with the others...so far.

Hypoxia and AMD

Hypoxia is a key trigger of AMD, and can be caused by a number of factors, including reduced blood flow to the retina, diseases like obstructive sleep apnea, and aging.

Drusen and hypoxia

Drusen deposits can create a relative hypoxic condition in the outer retina, which can damage the retinal pigment epithelium (RPE) cells. The RPE is essential for maintaining the photoreceptors, and damage to it can lead to visual loss.

Drusen and autophagy

Impaired autophagy, a cellular proteostasis pathway, can contribute to the accumulation of drusen. Autophagy can be stimulated by oxidative stress, which can interact with hypoxia to play a role in AMD pathogenesis.
Drusen and cholesterol metabolism
Disrupted cholesterol metabolism in the retina may be linked to drusen.

Drusen are deposits that contain damaged cellular proteins, lipids, carbohydrates, and lipoprotein particles. They can be caused by a number of factors, including a buildup of unwanted material in retinal cells, or when immune cells don’t function efficiently.

I never see that drug listed with the others...so far.

But there are some more serious side effects as recognized by the FDA. Serious side effects and their symptoms can include the following:

New or worsening kidney damage.
shortness of breath
swelling in your legs
Pancreatitis (inflammation in the pancreas).
Severe digestive problems.
Hypoglycemia (low blood sugar level).
Severe allergic reaction.
Risk of thyroid cancer.

None of the GL’s are completely safe. I’ve used jardiance, then ozempic and now trulicity becasue my body got used to the one I was using at the time and lost effectiveness. Nothing is perfect. And the only drug like it that is actually advertised and used as a weight control drug is wegovey but the dose in it is extreme and if you come off of it you will gain weoight rapidly under normal conditions.

Increased risk when T2D is present but what about when it is not?
