Baldwin’s set was a blatant slip-shod operation in which an accident of the sort that happened was bound to occur.

Baldwin shot someone because of a series of failures by everyone who handled the gun that day. If just one of them had been conscientious enough to check the gun to make sure it was a) unloaded if the scene did not call for firing it or b) properly loaded with blanks if the scene called for firing it. But Baldwin held the ultimate responsibility.
This incident was the fault of one person.
