That is a High Flow Nasal Cannula, which is commonly the last measure before going to Non Invasive Ventilation, i.e. CPAP/BIPAP. Last of course is intubation and mechanical ventilation.

This person very likely has a Sleep Apnea condition which makes breathing an issue, especially during slumber. His problems are mixed, maybe a swollen epiglottis or other sub glottic tissue. It is unlikely to be Central Sleep Apnea (a neurological condition) because he is so obviously overweight.

Give the guy a tracheotomy and that would probably get him breathing well.
