Two types of blood pressure meds prevent heart events equally, but side effects differ

In an analysis of almost 3 million patients taking a single high blood pressure medication for the first time, angiotensin receptor blockers (ARBs) were as good as angiotensin-converting enzyme (ACE) inhibitors at preventing cardiovascular events linked to hypertension, including heart attack, stroke and heart failure. 51 possible side effects and safety concerns were examined: The patients taking ARBs were found to be significantly less likely to develop tissue swelling, cough, pancreas inflammation and bleeding in the digestive tract.

While the class of blood pressure-lowering medicines called angiotensin-converting enzyme (ACE) inhibitors may be prescribed more commonly, angiotensin receptor blockers (ARBs) work just as well and may cause fewer side effects. Currently, ACE inhibitors are prescribed more commonly than ARBs as a first-time blood pressure control medicine.

Both types of medicines work on the renin-angiotensin-aldosterone system, a group of related hormones that act together to regulate blood pressure. ACE inhibitors lower blood pressure by blocking an enzyme early in the system so that less angiotensin, a chemical that narrows blood vessels, is produced, and blood vessels can remain wider and more relaxed. ARBs block receptors in the blood vessels that angiotensin attaches to, diminishing its vessel-constricting effect.

They found no significant differences in the occurrence of heart attack, stroke, hospitalization for heart failure, or any cardiac event. However, they found significant differences in the occurrence of four medication side effects. Compared with those taking ARBs, people taking ACE inhibitors were:

3.3 times more likely to develop fluid accumulation and swelling of the deeper layers of the skin and mucous membranes (angioedema); 32% more likely to develop a cough (which may be dry, persistent, and bothersome);

32% more likely to develop sudden inflammation of the pancreas (pancreatitis); and

18% more likely to develop bleeding in the gastrointestinal tract;

It might be time to look into the generic ARBs. Consider telmisartin, among others.

I can’t help but wonder how many people are diagnosed with hypertension erroneously.

Whenever I go to the doctor, dentist, or ophthalmologist, they say I have sky-high pressure. Even though I have no feeling of anxiety, it always reads high in those settings.

But at home, my blood pressure is perfect. And no, the machine is accurate. I’ve checked in the doctor’s office with my machine in front of him and showed him the memory with the at-home readings LOL.

I recommend videos by Dr. Sanja Gupta from York Cardiology. Read the comments below his videos some from medical staff.

I’ve tried 18 BP meds; all gave me bad side effects so I had to research alternatives like diet, exercise, deep breathing, magnesium powder, etc. I recommend everyone do their homework on every prescription med & know the side effects.

If you wear a tight belt, your blood pressure can shoot up.

It might be possible you don't wear tight clothing at home. Additionally, it could be you drink a lot of caffeine to get going.

It could also be white-coat hypertension.

Yeah, I thought it might be the morning coffee, and it does bump it up some. Just finished three cups and vaping nicotine all morning. So reading this I just checked and it’s a little high at 129/70. Damn, I’m old LOL.

same here. 130/85 at home almost every time. Doesn’t matter what doctor’s office I am in, it’s at least 170/110.

My body tends to hold fluid, which puts fluid in my inner ear, causing vertigo. Doctor put me on Hydroclorothiazide, which stops the body from absorbing salt. I am on the edge for vertigo but much improved and it helps my slightly high blood pressure too.

I run 120-25 over 78-85 at home, 139-90 at the doctor’s office. He think the BP is fine at 72 years old.

My blood pressure is lower at home. I am taking Lisinopril but would prefer not to due to side effects. I am pretty healthy and active. I don’t like prescription drugs. They tend to over prescribe when there are over the counter or other ways to treat. It is all about the money. They kept trying to over prescribe for my mother when in the nursing home.

Entresto and Covidol here. Because of a left branch electrical problem my heart beats twice for every one discharge valve opening. It’s hard on the muscle and the lower my pressure the better. Getting a synchronizing pacemaker in the fall to stop the electrical problem making the extra beat. Then I can wean off the meds.

I will add that from my readings, ARBs, followed by (or included with) a generic Calcium Channel Blocker are the preferred approaches to have the least side effects for most people.

Diuretics, for instance, are not considered a preferred approach, any longer.

Truth. My wife is diabetic among other ailments. Let’s just say thank goodness I have good retiree health insurance otherwise, I’d be broke. CVS doesn’t even need her name and birthdate half the time for pick-ups!

Here is a thought: instead of taking pharmaceuticals to paper over the symptoms try eliminating sugar and all processed foods and doing intermittent fasting so the underlying metabolic damage can heal? Worked like magic for me and fixed a number of other problems I took for granted as “getting older”.

Sugar is a toxin and we are not made to eat constantly on such an energy rich diet as our modern society provides. Replace carbs with fats and oils and it becomes easy to go 16 hours or more without eating.

I agree. The last two days I went over 20 hours each without any hunger and not wanting to eat at all. I felt fine and happy.

Low carb does wonders.

I’ve wounded it. Switched to vaping ten years ago after 35 years of heavy smoking. I make my own e-liquid and have just about cut out the nicotine completely.

See why vaccines don’t skeer me LOL. I’ve been rode hard and put up wet. But I do get my vitamin C from a daily dose or two or Blue Moon!

But you still have high blood pressure? Low carbs and intermittent fasting took my blood pressure down 25-30 pts, from the upper end of stage 1 hypertension to comfortably within normal even at the doctors office with no meds.
