My guess is that there was no GFI instant circuit breaker on the electrical outlets in her bathroom. Unlike the US, where such outlets are mandated.
It’s the amps that kill, not the volts,
But still, I doubt seriously you could be electrocuted with a USB charger.
And one has to wonder if they have the same circuit breakers we have here in the states.
Most likely the walwart part was on extension cord and went in water too

Your car battery is 12 volts. It will fry you quite effectively. On the other hand when you get a zap from touching a doorknob that's thousands of volts. Harmless.
Voltage is just a measure of potential energy between two points. Current is actual electrical charge moving around and that's what kills you if there's enough of it.
That said, I'd still find the charge coming out of a DC charging cable to be pretty unlikely to be sufficient to kill you.
You can't be electrocuted by the low voltage in a charger. But you can be electrocuted by a fault path from the AC line.
I can think of two ways for this to occur. The first is a fault through the insulation in the charger. Frankly, I think that is unlikely, because the chargers are built with two layers of insulation. Another way--the oops way--is if the charger was on an extension cord, and the extension cord fell into the bath with the charger. Then it's the same situation as if a live AC wire was placed directly into the tub. I wouldn't be surprised if that's what happened.
In modern houses, there are ground fault interrupter outlets near things that can provide an expedited path to ground... like near the kitchen or bathroom sink. There's no way to know if that was the case here.
My bet: The extension cord fell in with the charger.
A 12 volt car battery will kill you, because it's about 60 amps.
The charger that phone comes with is 1 amp, so I agree something is missing.
The batteries in our cars are 12 volt.

If there was no breaker and the charging wire was big enough to carry the required current, it could happen. But I am skeptical. I suspect foul play - the roommate would be my first suspect.
The problem with these modern switching power supplies is that the output is is not isolated from the input. So one side of the input is tied to one side of the output. If that common just happens to be the AC line voltage(Instead of the neutral). Then you have the potential to get zapped. The AC traveled through the water to ground. And this poor girl was in the path. An old fashioned charger with a transformer would have prevented this death , as would a GFI outlet.
Who in their right mind takes an electrical device into the bath with them?
