Link is from chicago.

I would lean towards lack of grounding or the infamous Chicago rats chewing up the ground cables.

RATS!!!!

Everything should have been in metallic conduit.

I have seen too many of the city rats, they might eat the insulation off the wire, but the conduit would defiantly slow them down.

“Much of ground straps are exposed.”

Not my line of work, but when my wife and I put a major addition on our house,we did all the work accept we paid to have the drywall stacked inside the house .
The only non trivial gig we received was a ground violation.

Back to the library and study the code.
Yes I had submitted detailed plans and schematics, that were approved.

We upgraded the service to 300 amp and made the old existing breaker box a slave to the new panel.

Had to run it in ONE INCH HEAVY WALL CONDUIT ( NOT THIN WALL ,NOT IMC)all the way from the old box to a new ground rod out side.
THIS WAS A PROJECT!
And don’t forget the THREE-TWO-BEE TAG, like I did.

This was of course residential and I’m in the next county over. Could all be different.
