After this many stupid incidents, a whole bunch of people need to be cashiered. Not just the CO, but lots of people above him.
I would go with decimation, as a policy. Shake up the entire institution enough that whatever this shit is that has taken hold, gets tossed out in institutional self-defense.

Having trans-GIBLETS in our military is the most important issue facing military readiness!
Being able to captain a combat ship not on the list of important requirements.
The other situation where a Captain is relieved of responsibility is when the ship transits the Panama Canal. The locks and transit pilots assume legal responsibility for the safe navigation of ship. Or it use to be that way when we operated the canal.
