I don’t think a civilized society requires a person to accept an offer of accommodation, only to be polite in either accepting or declining.
For example, I was carrying a load of camping gear to my car a couple of weeks ago, and two men offered to help. “No, thanks,” I said, “I’ve got it.” “I was just trying to be helpful!” said one, in an affronted tone. “That was nice of you,” I replied, “but I’m fine, thanks.”
They were supervising Scouts, while I was on a break. It was much more appropriate for them to stay in their roles, although the offer was kind. On other occasions, I was happy to accept assistance from both adults and youth in putting away the archery targets.

IOW, neither men nor women were likely to offer her a seat. Did she announce that she was pregnant, or were they supposed to be rude enough to presume it?

To demand that someone else be unselfish to you is itself a selfish act.
If you want chivalry, try saying Thank You when someone is chivalrous.
The women who lambasted men for holding the door open or giving up their seat made it worse for other women.

Here in the rural South it’s a bit different. Another gentleman and I tried to enter a fast food place at the same time. We each grabbed a door and insisted that the other go first. It was a standoff for a few seconds until I smiled at him a said, “I’ve got all day”.
He caved and went first, therefore I won. I love the South.
