OF COURSE a ticket purchase should be non-refundable. If a passenger is afraid they may not get to take the flight (for whatever reason), it’s a perfect example of what purchasing insurance should be for.

I've seen reports indicating that 10% of the reservations on any given flight will be changed after the tickets were initially purchased.

I'm not familiar with flight insurance, but my understanding is that it is really intended to cover a situation where you have to cancel a trip as opposed to rescheduling/rebooking a flight.
