I would guess that’s the hand of his controller, Valerie Jarett.
Perhaps some sort of operant conditioning mechanism in which she feels his head scar as a way to trigger some sort of behavior.

Well, it’s some old bag. I can tell, because I’m an old bag too. Old age spots first gave her away. Totally gray hair too. But she gets her nails done that’s for sure.
