I have read several postings about folks being fired for refusing to cover tattoos at work, my question is this, "does the employer have to have a written dress code policy and/or inform the employee at time of hire of his policy on tattoos, in order for the termination to be valid?"

No.
The individual would not be fired for having a tattoo. They would be fired for refusal to cover it while at work, just as they might be fired for refusal to discontinue any behavior the employer considered disruptive or bad for business.

My favorite tattoo is a line of dashes around the neck with "CUT ALONG DOTTED LINE" printed under it.
