Here is what I just used for my COVID-19 vaccine religious exemption / accommodation! I hope this helps someone out there. Note: I owe a debt of gratitude to Freeper PastorBillRandles' son for sharing his legal expertise in creating this...
Religion: Evangelical Christianity
Religious Objection: I have religious objections to the COVID-19 vaccine and all vaccines in general. As a Christian, I believe that my God protects us from deadly viruses and heals us when we are exposed to deadly viruses.
Specific Objection: My religious objections to the COVID-19 vaccine and and all vaccines is rooted in the earnest believe that my God protects us from deadly viruses and heals us when we are exposed to deadly viruses.
I also believe that all life is sacred and bears the image of God. Consequently, I believe that the act of abortion is deeply immoral. I cannot in good conscience receive any vaccine which uses, benefits from, is derived from, tested with, or is in any way connected to fetal cell lines. For me, to receive a vaccine associated with fetal cell lines would be to endorse and turn a blind eye to what I believe to be a grave sin.
As a Christian, I believe in the principle of bodily autonomy, bodily integrity, and informed consent. They are central to my Christian worldview. I am deeply opposed to any mandate that would result in the non-consensual penetration of my body as a condition of employment--especially in the absence of any long term, peer reviewed studies relating to potential side effects of the COVID-19 vaccines, or any vaccines.
Based on my sincerely held religious beliefs, I am my family have not received vaccines. Me, since becoming a Christian in 1992, and my 9 children since birth. I cannot in good conscience receive any of the currently available COVID-19 vaccines. Furthermore, I understand that the right to live in accordance with my convictions free of discrimination or coercion in this context is protected by a large, well-established body of law, including but not limited to the First Amendment to the United States Constitution, the Civil Rights Act of 1964, the Federal Food, Drug, and Cosmetic Act (and other federal law), as well as Iowa constitutional and statutory law.
Based on the foregoing, I am hereby requesting a reasonable accommodation and full exemption from any Covid-19 vaccine mandate
Accommodation: Regular testing, wearing a mask, social distancing, and/or working from home For the last 1.5 years I have been able to keep myself and my workmates safe by regular testing, wearing a mask, social distancing, and/or working from home.
Duration of Accommodation: I am requesting accommodation for the entire duration of this COVID-19 mandate.

Good thing you are not in Washington, there is no exemptions as the law. Check that, no law. The mandate is structured such that if you have ever been to a doctor, taken a pharmaceutical prescribed, or gotten a vaccination, you’re religion is not important to you...so says the King of Washington State.
In Washington state, religious exemptions were offered. Many given. But then (once all the religious state employees were identified), the exemptions were canceled on the grounds the agencies had no way of accommodating you. They did this with 2 days before the vax deadline!!
Employers legally have to make accommodations for physical and gender factors. I bet they are so obligated for Muslim and Indian religious based clothing too.. Just no accommodation for Christians.

You could say something as simple as per 1 Corinthians 6:19, my body is a temple of the Holy Spirit. At this time, unlike with many other medicinal compounds such as aspirin or tetanus shots, I have not observed enough clinical data over a long period of time to ensure that my temple will be maintained. Therefore I am uncomfortable taking these shots.

Most reasonable people wold find this lie hard to state. Unless you're one of those Earth Mother types who refuse to vaccinate their children against polio, DPT, and other childhool ailments you cannot claim this. Unless you lie.

I had a vaccine injury in the military and knew had friends whose healthy baby boys were rendered profoundly autistic after their 2 year shots. My oldest kids are in their 20s. Perfectly healthy and never vaxxed

I have been informed that the Department of Defense has directed me to get vaccinated with a COVID-19 vaccine, or suffer adverse employment action, up to and including termination. As such, I have made the decision to request a Religious Exemption Accommodation to the vaccine mandate pursuant to Title VII of the Civil Rights Act of 1964 (42 U.S.C. §2000e et seq.).
The reason for my request is based on my sincerely held belief in the sanctity of life. As a follower of Jesus Christ, I believe that life begins at conception and ends at natural death. Because fetal cell lines from babies murdered before their birth were used in the varying aspects of vaccine development, I cannot submit to your demand to be vaccinated nor am I required to do so under Title VII of the Civil Rights Act. This remains true whether the aborted fetal cell line has been synthetically replicated in a lab and no matter how many times it may have been replicated, no matter how tenuous the connection may be.
My proof that the COVID-19 vaccines were developed or tested using cell lines that were generated or derived from tissues of aborted fetuses is from the article linked below. J&J used an aborted fetal cell line in manufacturing its COVID-19 vaccine, while Moderna and Pfizer used aborted fetal cell lines in testing the efficacy of their vaccines. It is my point of view that an immoral act cannot become moral simply because the benefit received from the immoral act becomes tenuous. If it is immoral to kill an innocent human being in the womb then it is immoral to benefit from that killing in any way, no matter how tenuous the connection may be, even if the benefit is derived from a synthetic version of the human cell obtained from the aborted baby. Therefore, my religious belief that abortion is immoral and that I must have nothing to do with it no matter how tenuous the connection may be, is unmistakably “sincere” in nature. The Sixth Commandment prohibits murder, and since these vaccines are derived from murdered babies, to have them administered to me would make me complicit. I will not cross that line.
Federal law requires an employer to presume a Title VII religious accommodation request is sincere, absent objective evidence to the contrary. Title VII also requires the employer to provide accommodation so long as it is based on a religious belief and does not cause an “undue burden” that is significant and more than speculative.
I rely solely upon my sincerely held religious beliefs under Title VII in making this request for accommodation. It is for this reason alone that I cannot accept any of the vaccinations the United States government currently endorses.
In closing, I would like to emphasize my gratitude and appreciation for the working relationship we have maintained in my time working for the XXXXXXXXXXXXXXXXXXXX, the XXXXXXXXXXXXXXXXXX, and the XXXXXXXXXXXXXXXX, as a whole. Everyone has been kind, respectful and honorable and those facts have not been lost on me. I do not want to jeopardize my employment unless it requires me to abandon my sincerely held religious beliefs. As such, I hope to continue this working relationship under a reasonable accommodation.
