Actually it makes perfect sense. Your eye color is determined by the Iris surrounding the eye, be it brown, blue, green etc. All pupils are black. Under low light conditions the pupil expands to admit more light into the eye. Thus under low light conditions your eye appears darker or almost black as Joe’s.
Certain drugs such as catachol amines and amphetamines have the same effect. Such drugs would be useful to make a man suffering from dementia appear normal for brief periods of time.
Joe is stoked with drugs. Joe is not in control of the presidency.
About Joe squinting all the time, if he is on such drugs his pupil is dilated and admitting to much light onto the retina. This is just like when your eyes are dilated during an eye exam. Unless you have on dark glasses you will squint as to much light hurts.
There have been studies using salbutamol (an asthma drug) for Alzheimer.

A plausible choice. A cocktail of drugs may be at work, but with dementia patients, even when drugs help with the dementia, they tend to have side effects and lose their beneficial effects relatively quickly.

Yes. Catecholamines such as methamphetamine and indoleamines such as LSD dilate the pupils greatly.
Opioids make them pinpoint.

Do a search of Alzheimer’s disease and pupil dilation. One article reports research that when the brain is working hard, pupils dilate and that ophthalmologists are studying the rate at which pupils dilate in those suffering from dementia. It’s one way, if fact, to diagnose early Alzheimer’s disease.
Dilated pupils make his eyes look really black. Some medication also has that effect. I noticed those flat, black eyes back during the debates. It’s evidence something is wrong here.
People forget how to read as the dementia worsens. They are regressing back in time.
One of the early signs with my husband was that he stopped reading the daily paper, and no longer did the crossword puzzle.

I’m thinking of his Thanksgiving address when he pronounced “Psalms” as “palms.” He did it twice because he wasn’t comprehending what he was reading. It made no sense to him. He’s having difficulty getting through a speech. His face looks like he’s straining, he squints as if that will help his comprehension, and his voice conveys that this is a tremendously difficult task for him.
I’m thinking that soon he won’t be able to read from the teleprompter at all. Then what will his handlers do? Drugs will not cover this up.
Indeed. If excessive broadcasting light were coming towards his eyes, his pupils would contract and get smaller, showing more of the blue iris; not dilate and get larger, making his entire iris look black.
