Pelosi may have released those 39 to vote no against the socialized medicine bill after she knew that the Dems had enough votes. That way the 39 in the most conservative districts can keep there seats in the next election. They played that trick during the illegal alien amnesty bill. Fortunately, at that time there were enough Republicans to vote down the amnesty.
You never know with the Democrats, they can be quite two-faced.

Those 39 had political cover and were not needed to pass the bill. Three democrats from Tennessee (Davis, Gordon, and Tanner) all voted no. Heath Shuler, a democrat from North Carolina, voted no.
All of them had cover to vote no.
