Popular diabetes drugs may reduce the risk of dementia
Medical Xpress / Karolinska Institutet / eClinicalMedicine ^ | June 21, 2024 | Felicia Lindberg / Bowen Tang et al

People with type 2 diabetes who are treated with GLP-1 agonists have a decreased risk of developing dementia, according to a new study.

Drugs known as GLP-1 agonists or GLP-1 analogs have become increasingly popular in treating type 2 diabetes and obesity as they help control blood sugar, promote weight loss and protect the heart.

People with type 2 diabetes have an increased risk of developing dementia and it's been hypothesized that newer diabetes drugs such as GLP-1 agonists and DPP-4 inhibitors might have a protective effect.

In the new register-based study, researchers followed more than 88,000 older individuals with type 2 diabetes for up to ten years. Using a study design called target trial emulation, which imitates a randomized clinical trial, they analyzed the association between three diabetes drugs (GLP-1 agonists, DPP-4 inhibitors or sulfonylureas) and the risk of dementia.

They found that patients who used GLP-1 agonists had a 30% lower risk of developing dementia compared to those who used sulfonylureas, and a 23% lower risk compared to those who used DPP-4 inhibitors.

"This is important because it can help doctors make better decisions about which medicines to use for older patients with type 2 diabetes," says Bowen Tang, a Ph.D. student. "However, proper randomized trials are needed to establish with certainty that GLP-1 agonists reduce the risk of dementia."

So, which prescription drugs are they really talking about? What is the common name?

Drugs in this class include Byetta (exenatide), Victoza (liraglutide), Trulicity (dulaglutide), and Ozempic (semaglutide) among others.

Dementia/Alzheimer’s was clinically determined a few years ago to be a type of diabetes, sometimes called Type III Diabetes.

“Diabetes drugs in the GLP-1 agonists class are generally taken by a shot (injection) given daily or weekly and include:

Dulaglutide (Trulicity) (weekly)
Exenatide extended release (Bydureon bcise) (weekly)
Exenatide (Byetta) (twice daily)
Semaglutide (Ozempic) (weekly)
Liraglutide (Victoza, Saxenda) (daily)
Lixisenatide (Adlyxin) (daily)
Semaglutide (Rybelsus) (taken by mouth once daily)

These drugs mimic the action of a hormone called glucagon-like peptide 1. When blood sugar levels start to rise after someone eats, these drugs stimulate the body to produce more insulin. The extra insulin helps lower blood sugar levels.

Lower blood sugar levels are helpful for controlling type 2 diabetes. But it’s not clear how the GLP-1 drugs lead to weight loss.

Doctors do know that GLP-1s appear to help curb hunger. These drugs also slow the movement of food from the stomach into the small intestine. As a result, you may feel full faster and longer, so you eat less.”

But it is an illusion as the drugs reacte in the brain and can’t do any more than a common diet if the patient is willing. It also requires a regimin of exercise to complete the weight loss.

Extra insulin doesn't lower blood sugar if the cells have become insulin-resistant, right?

Best treatment for diabetes: Ketogenic diet and Intermittent Fasting.

But nobody gets rich off of that.

“Extra insulin doesn’t lower blood sugar if the cells have become insulin-resistant, right?”

You’re on the right track, but as of right now there is no consistency to diabetes or insulin resistance. What you eat, how much exercise you can get, stress, sleep amounts, psychological reactions, illness, use of certain drugs like steroids, and other related illnesses like heart disease or a past stroke or heart attack are just a couple of the things that effect the insulin resistance problem that includes diabetes type II.

So there are a lot of factors that contribute to the body’s inner workings in this area. This is why the numbers can change on the test sometimes within a short time like after meals and exercise. Up or down. And with it goes a different amount of insulin shot into the body for correction. It’s always a new day with this. Sometime sunshine, sometimes thunder storms. It’s an adventure.

Ozempic has significantly reduced my appetite. I feel full after eating half of what was on my plate and I continue to feel full most of the day.
I have lost 15 pounds in two months simply by eating less.

Yep, caloric intake is the major ingredient in weight control. I see heavy people jogging, over years, no weight loss at all.
