Has He had it checked out yet ? It could be any number of things so don’t go diving off the cliff into black water at night just yet.
I could list dozens of things that it COULD be !!!
Start with getting a medical check: Imaging/Ultrasound/etc.
Or just go with amputation, that’ll solve the problem.
Prayers up for your friend. I had severe pain in my left foot a few weeks ago. It was horrific pain just sitting, no way I could walk. It turns out to be rheumatoid arthritis. Praying your friend’s condition is manageable.
Did your friend take a fall? Possibly a hairline fracture of the tibia or fibula?
DVT maybe?
Is he a heavy smoker?
My friend had pain in one of her calves, and then the other. She has since had both legs amputated at the knees. It was Buerger’s disease as a result of extreme smoking since her teens. (Last week diagnosed with lung cancer.)
Is it a general, ongoing pain? In the muscle? (Cramping, possibly? Mine will when I’m asleep and I stretch my leg.) deeper in the bone? (My guess was a hairline fracture.) In the tendons? (Possibly a pulled muscle?) Could be a number of things. Definitely your friend should consult a doctor if it is unceasing.

I think it is likely the results don't fit the narrative, so the data is hidden or is no longer required to be reported. Early Covid was bad, but it wasn't even the top cause of death in my county. It came in 4th or 5th as the leading cause of death behind heart failure, cancer, and etc.
I had that several yrs ago. Turns out it was a clot. I eventually had to go on Eliquis because of my genetics...my dad died of thrombosis.
