A couple of questions relevant to this.
Anyone may answer.

1. Is a doctor >required< to include a diagnosis with a prescription?
2. Is the pharmacist legally required to ask the patients diagnosis when presented with the prescription?
3. Who is legally bound to answer the pharmacist, provided there is a legal to include/inquire a diagnosis for the presenting patient?

There is no national requirement. State boards may differ.

No no no, but as I’ve heard from some friends they had trouble finding a doctor to prescribe and then had trouble finding a pharmacy to fill it.
