1. Is a doctor >required< to include a diagnosis with a prescription?
2. Is the pharmacist legally required to ask the patients diagnosis when presented with the prescription?
3. Who is legally bound to answer the pharmacist, provided there is a legal to include/inquire a diagnosis for the presenting patient?

There is no national requirement. State boards may differ.

True. However, insurance companies will often require a diagnosis before they will accept a claim (on a Dr office visit, or a prescription).
Now, for IVM, that may not be an issue for many due to a relatively low cost (and therefore don't want/need their insurance involved). Just something to keep in mind.
