Made HR update the survey as in the last 2 questions they didn’t provide an answer for N/A I’m not getting the vaccine....so I told them either someone can’t finish the survey or they have to make a false statement.
They updated the survey. I answered it the same day they posted it, not because it’s any of their biz; but so I could stick a thumb in their eye.

1) Have you been vaccinated
2) Do you intend to get vaccinated
3) About when are you planning
4) Where would you prefer to do it
1) had a yes, partially, no
2) had an N/A if you were
3) asked for approximate time frames and an N/A if you already were, added N/A for not doing it after I emailed them.
4) had a list of possible locations (drs office, local clinic, onsite (they are looking into a potential onsite vaccination if enough people ask) again N/A if already and added an N/A for not doing it after I emailed.
Pretty short and to the point but your right there was no option for decline to disclose.
